Now your plastic doesn't have to look like every other business card out there. Create a credit card that's uniquely yours. Express your entrepreneur-style by uploading your own digital photos of your company headquarters or an employee group shot. This is a web-based service that you can use from any personal computer.

Identity Card

Place your photo on your credit card for extra security. This card can help protect employees and businesses from fraud if it is lost or stolen.

CASE's My Card Creation is only $15 per customized card. Your card will arrive within 7-10 business days. You must have an existing or approved CASE VISA card to order. Certain restrictions apply.²

How do I get started?
Once you have received your new card, register your card and activate your rewards after setting up your user name and password. Then start using your card and earning points!

How do I earn points?
You earn points for shopping at participating retailers. Be sure to check the UChoose website often to see what special offers you can purchase through the website to accumulate points. When shopping in-store, select credit and sign for your purchases.

Can my employees who are authorized cardholders earn points too?
Yes, employees sharing one VISA card loan with multiple cards are allowed to earn and pool points together in order to earn big reward items.

How much will it cost to participate?
There is no cost to you. Membership in UChoose Rewards is completely free to CASE Credit Union card holders.

When can I begin to redeem points?
Points accumulate during the month and become available the following month for redemption.

How long before my points expire?
There is no expiration. Use them for as long as you like.

For your convenience, CASE can issue your CASE VISA card the same day you apply and are approved at all of our branches. Simply visit any of our branches to pick up your new card³, right on the spot.

Required Application Documents

To ensure you receive an answer to your VISA loan decision as quick as possible, we suggest providing the following documents.

• Operating agreements for LLC's
• Partnership Agreements for Partnerships
• 1 year business tax returns
• 1 year personal tax returns
